Waiver Form instructions
Please read the following before using the waiver form:
Complete the waiver form in its entirety.
You must leave the waiver form at the home prior to the start of the business day — which is 8 A.M. — on the day the order is scheduled.
If you choose to leave the front door unlocked: Leave the form immediately inside the front door so the form will be visible to the service technician from outside the home.
If you choose to leave the front door locked:
Hide a key outdoors and give the key location to a Contact Center Representative prior to the date of service.
Leave the form either immediately inside the front door (so the form will be visible to the service technician from outside the home), or outdoors near the front entry.
Service personnel will not retrieve or return anything from the mailbox.
You do need a waiver form for unoccupied homes.
Service personnel will not enter if there are children under the age of 18 in the home, unless an adult is present.
Pets should be restrained or removed from the home.
Please remember to sign the waiver form.
